Incident Report
Records regarding disciplinary actions, work accidents and similar events that occur in the workplace can be kept on the system.
Path: HR > Other HR > Disciplinary and Other Procedures > Incident Report
Attention: Discipline and Other Procedures can be provided to the user from the Authorization Groups screen. Authorizations must be given for other transactions.
On the incident report listing screen, all incident reports registered in the system can be listed with the incident type, relevant person and institution, and date details.

Adding and Updating Incident Reports
Event reports are kept with many details. It is of great importance that the details given here are given correctly, as they may result in disciplinary action for the person who is negligent at work.

- Incident Type: The type of the incident is selected. As an example, work accident or inappropriate behavior can be chosen. You can edit the Incident Type parameter on the Control Panel > System > Parameters screen.
- Branch: The branch where the incident occurred is selected.
- Negligent: The employee who caused the incident is selected.
- Approval Date: The approval date of the incident report is selected.
- Explanation: Many details about the incident are given. In addition to written details, it is also possible to add visuals, videos and links in this field.
- Event Date: The date the event occurred is entered.
- Witnesses: People who witnessed the event in the company are added in this field. After the incident report is created, opinions of these people will be taken.
After clicking the "Save" button, the incident report is saved. Some details that can be added here about the incident report appear on the update screen.

Notification Letter to the Prosecutor's Office: If the incident is a matter of concern to the prosecutor's office, a notification letter to the prosecutor's office can be written with the details of the incident report.
Witness Statements: These are the witnesses entered on the saving screen. They can give their statements about the incident in this section. Note: Detailed witness statements are of great importance in keeping the incident report.
Defense Request Letter: A defense request letter can be prepared for the person who caused the incident.
Disciplinary Board Decision: The employee may be referred to discipline by making a disciplinary board decision as a result of the incident report. Here, details are given about the meeting, file, decision, chairman and members of the disciplinary board.
Termination Letter: A termination letter can be written as a result of the incident report.
Penalty Notification Letter:A penalty notification letter can be written.
After all the details are given, the incident report can be printed with the print icon at the top right or as a PDF. can be obtained.
